Brant Community Healthcare System (BCHS) is pleased to announce that Susan Pajor has accepted the role of District Stroke Lead effective May 22, 2015.

Susan is currently BCHS’s Stroke Best Practice Lead, and has been recently performing many of the duties of the Lead role to ensure that Stroke patients receive the best care possible.

In this role Susan will report to Trish Blancher, Group Leader in Acute and Transitional Care and will lead processes involved in the system development and sustainability of evidence based stroke care across the healthcare continuum for the District of Brant Haldimand and Norfolk Counties that includes but is not limited to pre-hospital, acute care, rehabilitation, outpatient care and community reintegration, for the Brant Community Healthcare System district stroke centre and the district of Brant Haldimand and Norfolk (BHN).

Through collaborative partnerships Susan will lead the development and implementation of a BHN district stroke work plan that supports the mission, vision and strategic directions of the Ontario Stroke Network. Accountabilities include the BCHS Outpatient Community Rehab Program and the Secondary Stroke Prevention Clinic.

BCHS will be posting to replace Susan as the Best Practice Lead early next week. In the interim, Susan and Trish will be sharing the duties of both roles.
