Complementary Vascular Health Primary Care Quality Improvement Toolkits to assist with quality improvement (QI) projects and plans are now available. A needs assessment with primary care providers determined the development of the Toolkits to help augment QI activities in primary care. The web-based Toolkits have been framed around patient engagement and feature key QI elements that can be contextualized to primary care practices, teams, and patients.
