This knowledge exchange session supports the sharing of initiatives for patient-centred & integrated vascular health best practices in primary care settings. Building from the Vascular Health Strategy Session in April 2014, you will have the opportunity to:

  • Review where we are at – receive a progress update, including a live demonstration, & provide input on the collaborative achievements and lessons learned related to the OSN sponsored primary care resources under development
    • Vascular Health Assessment and Support Tool (VHAST) – EMR embedded , patient-centred, decision support resource
    • Vascular Health QI Toolkit
    • Vascular Health Medical Directives
  • Discuss how these approaches link with current LHIN & other local related initiatives – inform & make the connection on these integrated approaches with Patients First: Action Plan for Health Care, Health Links and others. You will have the opportunity to share & discuss your experiences with other primary care stakeholders and take away knowledge and resources that could be adapted in your local setting
  • Inform where we are going – advise on future implementation & dissemination strategies for what is needed to sustain a patient-centred and integrated approach for best practice vascular health care delivery
  • Network with colleagues interested in vascular health best practice implementation of resources, services & programs within primary care

Target Audience:
Primary Care LHIN leadership, Primary Care Physician LHIN Leads, HQO, Quality Improvement Decision Support Specialists, OCFP Leads, Primary Care Organization Leads, Chronic Disease Network Leads
